About the Role:

We are searching for a highly motivated and organized Discharge Coordinator to drive a seamless discharge process for patients and families. This position is a key role in the care continuum and the patient experience and will work directly with internal providers, external providers, community resources, and patients/families. The ideal candidate will have education and experience in helping families navigate aftercare options.

Responsibilities

  • Assist with discharge planning and collaboration with treatment team members to complete the discharge in a timely manner. Attends and participates in teams meeting
  • Communicates effectively with patients, family, treatment team and community agencies and facilities to ensure continuity of care
  • Documents activity and education provided according to department/program standards
  • Educates patients and families on the discharge process. Focuses on positive discharge experience for all patients transitioning to the next level of care
  • Establishes aftercare services including referrals for outpatient providers, case management, transportation services, meal services, and peer support groups. 
  • Acts as a liaison with referral sources, employer/schools, EAP and community agencies to keep them informed of discharge plan
  • Reports on weekly discharged patients and investigates all discharges that are against medical advice
  • Makes follow-up contact with patients and families at specific post-discharge intervals
  • Requirements

  • Bachelor’s degree
  • 3+ years of experience in a healthcare setting, preferably in care management, case management, or social work role 
  • Ability to receive numerous pieces of information and synthesize it into coherent storylines about the patient journey through treatment 
  • Strong communicator with a high level of confidentiality
  • Ability and willingness to pivot to other potential tasks
  • Enthusiastic, strong team player a must
